Malegra Oral Jelly by Sunrise Remedies Pvt. Ltd.

Malegra Oral Jelly 100 mg contains sildenafil citrate (SC) which is mainly used for treating erectile dysfunction (ED) in men. It comes in orange flavor only, unlike its counterparts which have multiple flavors. Oral jellies are one of the new forms of oral administration for sildenafil, as manufacturers are exploring more options for its oral delivery as there are a number of concerns for the film-coated tablet form. Oral preparations which are quickly disintegrating are made for faster absorption and fast action for SC [1].

This product, Malegra Oral Jelly 100 mg is only one of the available Malegra products made by the manufacturer Sunrise Remedies Pvt. Ltd. This product line is one of its ED product lines and Malegra, in particular, involves sildenafil-containing ED products [2]. Sunrise Remedies has different variants for its SC-containing ED products like oral jellies, hard pills, and combination pills containing SC plus premature ejaculation treatments like dapoxetine and fluoxetine. These Malegra products are widely available on the Internet, as a large number of online pharmacies distribute this product line worldwide.

Sunrise Remedies is another India-based pharmaceutical manufacturer (Ahmedabad) and exporter which started almost 30 years ago. It is an ISO 9001:2000 certified company which is primarily known for its Herbal and Ayurvedic remedies [3]. But aside from its herbal products, the company is also well-known for its generic products and its sexual enhancement products in particular (mostly erectile dysfunction generics). Sunrise Remedies is also known for its Malegra products, which are generic substitutes for the expensive brand-name sildenafil citrate-containing treatments for erectile dysfunction. It also has a separate product line for other types of ED meds, like for instance, Tadarise which contains tadalafil.

The company also has products for cognitive neuropsychiatry, cardiac care, brain activators, anti-epileptic meds, vertigo meds, Parkinsonism treatments, and weight control tablets [3]. It is said that the company’s strong suit is its research and development, as it has one of the biggest R&D departments in India [4]. However, there is not much information regarding the company’s approvals when it comes to US FDA and WHO-GMP approvals. Being a company running for almost 3 decades, though, its Indian FDA approval is implied. But there was no clue whether the company gained approvals from the esteemed international drug regulating agencies.

 Studies Carried on Sildenafil Citrate

Sildenafil citrate as a treatment for erectile dysfunction (ED) in men has long been approved by the US Food and Drug Administration (March 2008) and since then, it has been used as the first line of treatment for ED. Sildenafil’s use and effectiveness over the past years have been evaluated by the paper “Sildenafil citrate: a therapeutic update” by Boyce and Umland [5]. According to the data presented in the research, SC is an effective treatment for men with ED and for ED coinciding with a number of medical conditions like prostate cancer (patients after prostatectomy), diabetes mellitus, ischemic heart disease, spinal cord injuries, and neurologic disorders. Sildenafil citrate (SC) has not been effective in treating sexual dysfunctions in females, though, but SC has seen to have effect in female sexual dysfunctions associated with selective serotonin reuptake inhibitors (SSRIs) [5]. It was also discovered that aside from ED, sildenafil was also found to have a significant effect on non-ED cases like esophageal motility problems. However, even if SC was found effective in treating the general population with ED, it has been ill-advised for patients with cardiovascular diseases (CVD) or those under nitrate medication [5]. Also, aside from patients with CVD on nitrates, patients under cytochrome P450 3A4 isozyme inhibitors are also not advised to undergo SC treatment as the mentioned isozyme metabolizes sildenafil and may exacerbate its effects and may cause toxicity for the patient even for normal SC doses [5]. But all in all, SC is still the first-line therapy for ED but it is recommended to consider a patient’s medical history and the cost-effectiveness of the treatment before prescribing the drug [5].

Studies delving deeper into the use of sildenafil for ED cases brought about by other medical conditions like ED post radiotherapy for prostate cancer were also conducted in the past years. One of these studies, “Efficacy of sildenafil in an open-label study as a continuation of a double-blind study in the treatment of erectile dysfunction after radiotherapy for prostate cancer” made by Incrocci et.al., studied whether SC is also effective in treating ED caused by radiotherapy for prostate cancer [6]. SC treatment for ED of the mentioned etiology proved effective, as there was a significant increase from the baseline data. Initial dosing for the patients was 50 mg, but in the case of an unsatisfactory response from the patients, the dose was increased to 100 mg [5]. Researchers concluded that based on the response by patients treated with SC, the drug is relatively effective in treating ED post radiation for prostate cancer. They recommended adjunct treatment, though, to improve the results [6].

Another study, “Incidence of erectile dysfunction and characteristics of patients before and after the introduction of sildenafil in the United Kingdom: cross sectional study with comparison patients”, by Kaye et.al., sought to evaluate the effectiveness of sildenafil based on the response of patients from the United Kingdom [7]. The result of the study determined that the discovery of sildenafil treatment doubled the cases of erectile dysfunction in the United Kingdom, but the result did not necessarily mean that patients with ED multiplied. It just meant that patients were made aware of their ED condition upon the discovery of the drug too (in UK patients, at least) [7].

Sildenafil Citrate Role in the Erection Response

Sildenafil citrate belongs to the family of PDE5is (phosphodiesterase type 5 inhibitors). SC specifically inhibits PDE5 from degrading cGMP, as PDE5 binds with cGMP and regulates its intracellular levels (Inhibition of cyclic GMP-binding cyclic GMP-specific phosphodiesterase (Type 5) by sildenafil and related compounds” by Turko et.al. in 1999) [8]. The action of SC in blocking PDE5 causes the cGMP levels to accumulate in the corpus cavernosum. This increased levels of cGMP leads to relaxation of the smooth muscles of the corpus cavernosum of the penis and promotes vasodilation [8]. Not only does SC promote improved erectile response, but it also helps sustain the erection due to its PDE5-inhibiting activity.

Effects of Sildenafil

Sildenafil’s effect (when used for ED treatment) is primarily the improvement of the patient’s response to sexual stimuli. Patients suffering from ED when treated with any sildenafil citrate-containing medication can expect to have improved erectile response and prolonged erections.

Since Malegra Oral Jelly contains sildenafil citrate and in the most effective dosing (100 mg), patients may expect to attain erections in response to sexual stimuli. Some vendors use the term generic Viagra for Malegra Oral Jelly [9], so patients can expect the same results from the product as it contains the same active pharmaceutical ingredient (API).

Recommended Doses and Duration of Therapy

Sildenafil treatment for ED is actually patient-condition dependent, as there is no really recommended dose effective for everyone. But, the approved dosages for SC in the market are 25 mg, 50 mg, and 100 mg. In some clinical studies for the effectiveness of SC in ED treatment, the initial dose given to patients is 50 mg and then adjusted when the dose seemed ineffective [6].

Malegra Oral Jelly, though, contains 100 mg of sildenafil citrate in every sachet and as per manufacturer’s instructions it may only be taken once within 24 hours, and as needed only [9]. Malegra Oral Jelly should also be taken 45 to 60 minutes before the intended activity and is not recommended to be taken with alcohol and fatty foods as they may delay the drug’s effect [9].

Side effects and Warnings

Sildenafil is commonly associated with side effects like headache and flushing, and the oral film-coated hard pill sometimes causes dyspepsia and heartburn. Possible side effects of sildenafil as reported by some patients during clinical testing include anaphylactic reactions, difficulty in breathing, hearing changes, vision changes, chest pain, tachycardia, arrhythmia, prolonged and painful erections (for more than 4 hours), diarrhea, indigestion, stuffy nose, and many others [9]. Side effects vary from one patient to another so these side effects may not be the only ones which may be observed in SC users.

Drug Interactions

The major interactions noted for sildenafil citrate is its interaction with organic nitrates, so patients with cardiovascular diseases and on nitrate medication are generally prohibited to take the drug (“Pharmacology and drug interaction effects of the phosphodiesterase 5 inhibitors: focus on α-blocker interactions” by Kloner) [10]. Other medications contraindicated for SC are blood pressure meds, HIV/AIDS medication, fungal/yeast infection drugs, and other sildenafil-containing drugs [9]. It is important that patients are able to disclose all of their medications during the assessment, even herbal remedies, and supplements, as some of these drugs are also known to interact with any medication containing SC, and in this occasion, Malegra Oral Jelly.

Conclusion and Rating

Since Malegra Oral Jelly also contains sildenafil citrate, its effectiveness may be assured, as studies regarding sildenafil citrate use present how effective SC is in treating the dysfunction. Malegra Oral Jelly is one of the famous products manufactured by Sunrise Remedies in India. Based on the user comments, Malegra is effective as it was well-received by the online buyers. Reviews were also mostly admiring the product’s good taste, as Malegra Oral Jelly’s orange flavor seemed to be delicious from the users’ perspectives as well.

Although there is much admiration for the product’s effectiveness, I can only rate Malegra Oral Jelly 3 out of 5. It is an effective ED treatment for patients, but its manufacturer, Sunrise Remedies, fell short of approvals like WHO-GMP and US FDA accreditations. Sure, the company was multi-awarded, but there is not much concrete information regarding its manufacturing standards and process. But since the users were mostly happy with their online purchase, I think Malegra Oral Jelly is still worth the try.
